By completing this form for yourself or as the parent/legal guardian of a child, you are assuming full responsibility for the selection, use, and return of all materials borrowed and for charges on items that are overdue, damaged or lost.
Upon completion of this form, you will receive an email containing your library card number and personal identification number (PIN) after which your physical card will be mailed to you. Please sign the back of the card when it is received. You may be required to confirm your name and address by showing an acceptable form of identification upon your first visit to the Library, such as a driver’s license or a piece of mail.
Please do not complete this form if you already have a library membership but your card has been lost or damaged. In the case of a lost or damaged card, please email library@haldimandcounty.on.ca to receive a replacement membership number and card at a $2.00 cost.
